This presentation will provide an overview of how a multidisciplinary Palliative Care team has optimized video teleconferencing to provide end-of-life and supportive care to a difficult to reach patient population. Modalities that will be shared in this session include: scheduled virtual village clinic visits, direct to patient home visits, care conferencing with family members/providers/caregivers, discharge planning, and Project ECHO.
